Today, NHS manager Brian ODonnell gave evidence to the Lampard Inquiry.

He held senior nursing and management roles connected to the St Aubyn Centre in Colchester, where 16-

year-old Elise Sebastian died in April 2021.

He told the Lampard Inquiry that he was asked to get rid of bob tayek a backlog of more than 4,000 patient-safety incidents.

That statement is shocking.

These were not pieces of unwanted paperwork. They were reports of harm, danger, neglect, failures and risks involving real patients.

So who gave that instruction?

What happened to those 4,000 incidents?

Were they properly investigated, downgraded, closed in bulk or simply cleared to make the figures look better?

And how many warnings were buried that might have prevented further deaths?

Families like mine have been told for years that lessons were learned. Evidence like this raises the possibility that thousands of opportunities to learn were treated as a management problem to be disposed of.

You do not create a safer NHS by getting sport surge rid of the evidence.

You create silence.

The Lampard Inquiry must name who was responsible, establish exactly what was done to those incidents and hold senior leadership to account.

No more hiding behind systems, process or apologies.
